Clarification and extension of the terms of Form 9465. . If the total amount you owe is more than $25,000, but no more than $50,000, you must complete (1) lines 13a and 13b and agree to direct debit payments, i.e. (2) activate Box 14 to make your pay deduction payments and attach a completed and signed Form 2159. A salary deduction agreement is not available if you submit Form 9465 electronically. If your new monthly payment does not meet the requirements, you will be asked to review the amount of the payment. If you are unable to provide the minimum payment required, you will receive instructions to complete a PDF file information form for the collection information statement and for transmission. As a general rule, we will notify you of the authorization or refusal within 30 days of receiving your application. However, if this request is due, if you receive a tax return that you filed after March 31, the response may last more than 30 days. Once a missed contract has been approved, you can apply to amend or terminate a tempered contract. You can change your payment amount or due date by IRS.gov/OPA.
